Med-IT: Behavioral Health Integration

It has been proven that combining general care with behavioural and mental health care improves patient outcomes. The Centers for Disease Control and Prevention recognizes that mental and behavioral health care are linked, and Medicare pays physicians and non-physician practitioners separately for BHI services. Our BHI service is part of the FQHC/RCH G0511 codes and supports CMS code 99484.


Patients in our BHI program will benefit from ongoing relationships with our health coaches and care team, as well as individualized care planning tailored to the patient's own goals and interests in relation to their specific health problem, as well as facilitation and coordination with their primary care provider.

CORE COMPONENTS

Through our telephonic and digital applications, specially trained Health Coaches provide behavior modification help, motivational interviewing, a patient-centered approach to goal setting, and increased care support.

Patients will go through one of our exclusive behavioral health modules, which have been designed by a top behavioral health practitioner and are guided by our team of Registered Nurses.

Personal Activation Techniques are used by our behavioral health coaches to deal with patients.Shared decision-making, wellness activities, and self-management strategies are examples of patient engagement and activation initiatives.
